Appointments

We know you have many choices when choosing an dermatologist so we have made requesting an appointment a simple and convenient process via our Web site. If, for any reason you cannot keep a scheduled appointment, 24-business hours notice is requested. Less than 24-business hours notice may incur a $25 charge.

Co-Payments and Referrals

In compliance with your insurance guidelines, all applicable co-payments will be collected at teh time of the visit. There is a $25 administrative billing fee that will be charged fro any co-payments that are not made at the time of the visit.

If a referral is required by your insurance company, it is your responsibility to obtain and track the number of visits and expiration dates on your referrals. Kinnelon Dermatology is not authorized to treat patients who do not have a valid referral in place at the time of service. We ask that you bring your referral with you to the visit, however if your Primary Care Physician wishes to fax the referral to our office, please verify it has been received in the office at least 24 hours prior to your appointment. Unfortunately, if we do not have a referral at the time of your visit, we will be unable to see you and the appointment will need to be rescheduled. Our fax number is 973-492-2858.

Insurance Cards and Identification

Insurance cards must be brought with you to each office visit. If you have recently obtained insurance and will have your visit prior to having received your insurance cards, please print out the temporary information from the insurance company's website. Should you be able to produce proof of insurance, you will be required to pay for the visit at the time of service and submit the claim to your carrier for reimbursement

In order to protect patients against identity theft, the Federal Government requires all Medical offices to obtain proof of identification on patients when using insurance cards. Please have photo identification such as a drivers license with you at the time of your visit for this verification.
